Hot Topics in Myeloma
Recent developments in multiple myeloma include the FDA approval of linvoseltamab, a bispecific antibody targeting BCMA with a 70% response rate in heavily pretreated patients. Ongoing debate surrounds belantamab mafodotin, an antibody-drug conjugate with promising efficacy but notable ocular toxicity, currently under FDA review despite approval abroad. Minimal residual disease (MRD) testing is gaining prominence as a decision-making tool, with trials like MIDAS evaluating MRD status to guide treatment intensity. Long-term follow-up from the CARTITUDE-1 trial revealed that one-third of patients receiving cilta-cel remained disease-free at five years, raising critical discussions on how to define cure in multiple myeloma. Collectively, these developments highlight significant progress toward durable remission and potential pathways to cure.
