ATLAS: A phase 3 randomized trial of carfilzomib, lenalidomide, and dexamethasone versus lenalidomide alone after stem-cell transplant for multiple myeloma
The aim of Phase 3 of the ATLAS Trial is to directly compare the benefit of post-autologous stem cell transplantation (ASCT) treatment with carfilzomib, lenalidomide, and dexamethasone (KRd) therapy to standard lenalidomide (R) maintenance. The primary objective was to compare progression free survival (PFS) rate between the two arms.
Dr. Dominik Dytfeld (Medical University of Poznań—Poznań, Poland) discusses Phase 3 of the ATLAS Trial with newly diagnosed multiple myeloma (NDMM) patients receiving any induction therapy for up to 12 months, followed by single ASCT and achievement of at least stable disease within 100 days afterward. Patients were randomized to receive either KRd or R, stratified by post-transplant response and cytogenetic risk.
This is the first randomized phase 3 trial demonstrating superior progression-free survival (PFS) with extended post-transplant KRd therapy compared to R maintenance. Therefore, Minimal Residual Disease (MRD)/risk-adapted post-ASCT extended KRd treatment may represent a new standard of care.
