KRd with or without daratumumab in newly diagnosed multiple myeloma: ADVANCE trial results
The ADVANCE study compared DKRd (daratumumab, carfilzomib, lenalidomide, dexamethasone) with KRd (carfilzomib, lenalidomide, dexamethasone) in newly diagnosed multiple myeloma patients. Primary endpoint was MRD negativity, showing DKRd significantly outperformed KRd (59% vs 36%). Safety profiles were comparable, with DKRd demonstrating a higher MRD negativity rate without new safety concerns.
Key Points:
Research Problem/Question: Compare DKRd vs KRd efficacy in NDMM.
Methodology: Randomized 306 patients to DKRd or KRd, assessing MRD negativity and safety.
Key Findings: DKRd achieved significantly higher MRD negativity (59% vs 36%) with comparable safety profiles.
Significance/Impact: DKRd could set a new standard for initial therapy in NDMM.
Conclusion:
The ADVANCE study supports DKRd as a superior treatment option for NDMM based on significantly higher MRD negativity rates compared to KRd, with ongoing evaluation of survival outcomes.
