"Apparent diffusion coefficient histogram parameters of the whole tumor based on whole-body magnetic resonance imaging can better characterize the tumor burden of multiple myeloma and predict survival prognosis"
Source
Dong H, Qi Q, Ji X, Gao S, Zhang K, Liu S, Song Z, Huang W, Xia S. Apparent diffusion coefficient histogram parameters of the whole tumor based on whole-body magnetic resonance imaging can better characterize the tumor burden of multiple myeloma and predict survival prognosis. Quant Imaging Med Surg. 2026 Oct 1;16(10):782. doi: 10.21037/qims-2026-0524. Epub 2026 Sep 11.
Overview
Researchers examined whether whole-body MRI measurements of water movement across all tumor sites could better estimate disease burden and help predict survival in newly diagnosed myeloma. In this retrospective study of 95 patients, whole-tumor measurements correlated with bone marrow plasma cell levels, and several imaging measures were associated with survival. The approach needs further validation before routine use for prognosis.

Overview
This preclinical study tested whether blocking myeloperoxidase, an enzyme that influences the bone marrow environment, could slow myeloma or delay its return. In mouse models, AZD5904 reduced early tumor growth and delayed relapse when given after bortezomib, but did not help established disease when started alone. These results support further investigation of maintenance treatment, with benefit in patients still unproven.

Clin Cancer Res 1 October 2026; 32 (19): 4474-4488. https://doi.org/10.1158/1078-0432.CCR-26-0752
Overview
Researchers investigated whether changes in gut bacteria and bile acids contribute to diarrhea during lenalidomide maintenance. Samples from two prospective trials linked diarrhea to reduced bacterial diversity and altered bile acid processing. Differences present before treatment also suggested possible risk markers. The findings support research into microbiome-based approaches but do not establish that changing gut bacteria prevents diarrhea.

Overview
This long-term MAIA analysis examined whether the benefits of adding daratumumab to lenalidomide and dexamethasone differed by frailty. Progression-free survival and deep responses improved across frailty groups. Overall survival benefit was established in non-frail patients, while the estimate in frail patients remained statistically uncertain. Frailty was assessed retrospectively, which limits the subgroup conclusions.

Overview
Researchers investigated whether blocking SETD2, an enzyme involved in gene regulation, could target myeloma resistant to immunomodulatory drugs or CELMoDs. Resistant cell lines were more sensitive to SETD2 inhibition, particularly in a model carrying t(4;14). These preclinical findings support further drug development, but the proposed strategy has not been validated in patients with resistant disease.

Overview
Researchers examined whether CAR T-cell expansion could help identify patients at risk of delayed neurologic toxicity. Rapid, high expansion after cilta-cel was associated with these complications, and routine lymphocyte counts offered a practical surrogate in a larger cohort. The findings suggest a monitoring approach, but the proposed thresholds and preventive interventions require further validation.

Overview
This post hoc analysis investigated why patients with t(11;14) myeloma have differing outcomes. Among 482 assessed patients, the t(11;14)-positive group had fewer deep responses and distinctive survival and cell-marker patterns. Exploratory treatment associations varied by marker subgroup. The study challenges treating all t(11;14) cases as clinically uniform and calls for prospective validation of subgroup-specific approaches.

Overview
Researchers reviewed 94 older, transplant-ineligible patients receiving dose-adjusted daratumumab combinations to compare the prognostic value of frailty, low muscle mass, and fat within muscle. Greater muscle fat was associated with poorer survival, while frailty and low muscle mass alone showed no statistically conclusive association. The findings support further study of muscle quality alongside standard assessments.

Overview
Researchers used intensive-care records to assess whether blood urea nitrogen levels at admission were associated with mortality in critically ill patients with myeloma. Higher levels were linked to greater short- and longer-term mortality after adjustment. The findings suggest a readily available risk marker, but do not show that lowering this laboratory value itself improves survival.

Medicine, 105(42), e51073. https://doi.org/10.1097/MD.0000000000051073 October 16, 2026.
Overview
Researchers examined whether myeloma cells circulating in blood could reflect disease burden in newly diagnosed patients. In a small retrospective cohort, higher cell levels were associated with more marrow involvement, lower hemoglobin and albumin, and higher beta-2-microglobulin. The findings support further evaluation as an additional baseline assessment, but the study did not establish an ability to predict survival.

Cancer Epidemiol Biomarkers Prev 1 October 2026; 35 (10_Supplement): B064. https://doi.org/10.1158/1538-7755.DISP26-B064
Overview
Researchers examined whether genetic ancestry and the Duffy-null genotype were associated with response, survival, or transplant outcomes in a diverse cohort of 279 patients. Duffy-null status was linked to lower odds of a deep response, but neither genotype nor ancestry independently predicted survival. Strong overlap between ancestry and genotype limits causal interpretation, and larger studies are needed.

This phase 1 trial tested the safety of simultaneously administering separate BCMA- and GPRC5D-directed CAR T-cell products. Nine patients received both products, with a 78% response rate, but an inflammatory complication occurred and biological analyses suggested competition between CAR populations and persistent antigen-loss resistance. The small study supports further optimization and cannot establish superiority over single-target therapy.

Overview
Researchers studied 598 cilta-cel recipients to identify high-risk features associated with earlier or later treatment failure. Extramedullary disease, poorer physical performance, and prior BCMA therapy were associated with failure within six months, while early frontline relapse and high-risk genetics were associated with later failure. The findings may inform counseling and follow-up, but risk-based treatment changes need prospective evaluation.

Future Oncol. 2026 Oct 12:1-12. doi: 10.1080/14796694.2026.2720305.
Overview
This plain-language article explains long-term CARTITUDE-1 results after a single cilta-cel infusion in 97 heavily pretreated patients. At least five years later, 32 participants remained alive without progression or additional myeloma treatment, and median overall survival was 60.7 months. The purpose is to communicate evidence of durable remission in some patients, without establishing that the treatment cures myeloma.

Overview
This randomized phase 3 trial in China tested whether adding felzartamab to lenalidomide and dexamethasone improved disease control in relapsed or refractory myeloma. Median progression-free survival improved from 10.4 to 18.9 months, but severe blood-count abnormalities, infections, and treatment-related deaths were more frequent with the combination. Participants were not lenalidomide-refractory, which matters when interpreting applicability.

EClinicalMedicine. 2026 Oct 2;100:104208. doi: 10.1016/j.eclinm.2026.104208.
Overview
The PROMISE South Africa study screened 2,004 Black adults in Soweto using mass spectrometry to estimate MGUS prevalence and explore associated exposures. Heavy-chain MGUS was detected in 12% and light-chain MGUS in 3%. HIV and smoking were associated with higher prevalence. The single-visit design cannot establish causes or progression risk and supports further longitudinal research.

Overview
Researchers measured soluble PD-L1 before and six months after autologous transplantation in 40 patients to assess its relationship with outcomes. Higher levels were associated with poorer survival and relapse, and higher post-transplant levels with graft failure. The study explores a potential risk marker, but its small size, short follow-up, and multiple unadjusted comparisons make the findings preliminary.

October 2026.
Overview
Researchers investigated whether CD200, a protein already linked to immune suppression, also directly promotes myeloma-cell growth. Experimental findings showed growth effects dependent on interaction with the IGF-1 receptor, and high expression of both proteins was associated with poorer patient outcomes. The work suggests a potential combined treatment target, with therapeutic benefit requiring further investigation.

October-December 2026.
Overview
Researchers reviewed 40 patients who received fresh rather than frozen stem cells for autologous transplantation to assess feasibility, recovery, and toxicity. All achieved engraftment, and no infusion-related adverse events were observed. The approach avoided the freezing preservative and related resource use, but the uncontrolled study cannot establish faster recovery, lower costs, or superiority over standard cryopreserved transplantation.

Overview
This commentary examines gaps between advances in myeloma treatment and access within Brazil's public healthcare system. Drawing on registry and published data, the authors describe survival disparities, delayed availability of newer medicines, and barriers to essential diagnostic tests. Its purpose is to argue for more equitable implementation of effective care, including supportive services, rather than report a new randomized study.

Overview
Researchers tested whether attaching radioactive iodine-131 to tocilizumab could strengthen its antimyeloma activity beyond blocking the interleukin-6 receptor alone. The labeled antibody inhibited myeloma-cell growth in laboratory experiments and suppressed tumors in mice, including models not responsive to interleukin-6 signaling. The findings support further preclinical development; safety and effectiveness in patients remain unestablished.
